Simplified Clinical Tools and Educational Outreach for Health Workers

Developed in South Africa, simplified tools and training have been shown to improve outcomes for HIV, tuberculosis (TB) and primary care patients, and to enhance staff satisfaction and confidence. The following are key to their success: point-of-care training provided onsite in a clinical setting; case-based content; intermittent training sessions facilitated by peer trainers over time; and sustained educational support thereafter. This project will adapt the approach used in South Africa for Malawi, a more fragile health system where the challenge of integrating HIV services within primary care services is even greater. Working closely with the Ministry of Health and using a train-the-trainer model, researchers will test the approach, Simplified Tools and Training - Practical Approach to Lung Health, Malawi (STAT-PALM), in 31 rural health centres in Zomba district using a cluster-randomized trial. The training will focus on mid-level healthcare workers, that is, nurses, medical assistants and clinical officers. Outcomes measured will include staff retention and satisfaction, and patient outcomes with respect to HIV/AIDS, TB, malaria and other conditions seen in primary care. The overall goal is to improve access to high-quality HIV/AIDS care and primary care services.
